Rotochopper is offering employees the opportunity to earn $500 in 2022 as part of our focus on employee well-being and preventative care. Just complete the following activities by 11/30/2022 to earn $500 HSA dollars!
- Medical Preventative Exam Form
- Dental Preventative Exam Form
- Tobacco Affidavit
- Health & Wellbeing Activity (via Health Partners Wellbeing site)
- Health Assessment (via Health Partners Wellbeing site)
Return any completed activity forms to HR. Earned HSA dollars will be added to your HSA account at the end of 2022.
